Through all of the drastic changes we have experienced during the COVID-19 pandemic, one thing has remained true: everyone must be prepared for an unexpected and serious illness.

This toolkit has been created to be used by facilities and organizations who care for people, especially seniors and older adults. These tools are intended to be shared with and used by staff, as well as residents and family members to discuss Advance Care Planning and to ensure understanding and support through decision making that may be difficult.
