The Fox Valley Advance Care Planning Partnership is a collaboration of ThedaCare, Ascension NE Wisconsin, Mosaic Family Health, and the greater Fox Valley community working together to make advance care conversations and planning an integral part of living. For 20 years, professionals in our community have been working together to change the culture around death and dying. As we know, these are universal experiences for which many individuals and families are not prepared to discuss or plan. The Partnership, formed in 2016, serves as a catalyst for community action and ensures collaborative, community-focused work that benefits all.
