Who would you want to make those decisions for you?
How would they know what you want?
Advance care planning starts with considering your values and preferences for health care. Be sure to have conversations with your loved ones, so they know what you want. Complete the document, an advance directive, with one of our facilitators to make it legal, at no cost. Share it with your loved ones and healthcare providers.
In Wisconsin, every adult, age 18 and older needs an advance directive.
