What is Advance Care Planning?
Imagine you are suddenly unable to make medical decisions for yourself. Who would you want to make those decisions for you? How would they know what you want?
Advance care planning is an ongoing conversation of understanding, reflecting on, and discussing future medical decisions and preferences, should you be unable to speak for yourself.
It's important to document your preferences using a written advance directive, but this is just the start! Make sure your wishes are well known by talking about them often with your loved ones and doctors.
