Content was created by the U.W. Extension - Madison program. Each participant recieves a book, a thumb drive to record your personal/private information and expert guidance. Normal price for this class is: $40 per participant but Hortonvill class is free due to sponsorship from Fox Valley Advance Care Planning Partner
