FVACPP offers:  Free State of WI legal documents, education, and facilitated conversations about health care choices that a person would want if they were unable to make their own decisions in a serious illness, dementia, accident, or end-of-life.  Any one 18 years or older in WI needs an Advance Directive/Power of Attorney for Health Care.  We are passionate about normalizing conversations about serious illness and empowering your conversations with your important people so you recieve the health care you want.    This includes your Voice when you are not able to direct your care and it gives greater peace of mind to those most important to you (reducing self-doubt, prolonged guilt, and emotional trauma about whether they made the 'right' decision for you or did what you would want.)  Your agent could be a trusted person (non-family), chosen family, friend or other that will ensure you recieve the health care that fits with your unique values and preferences.
