Community Experts Helping to Organize & Complete Personal Wishes
Let's Talk: Advance Directives and Beyond is a multi-part program discussing personal wishes for care at the end of life and after death, and how people can ensure those wishes will be honored. Experts from various fields educate, share resources, and answer questions to help participants learn, feel more comfortable with the topics, and take action to complete important documentation, including legal documents.
The program is generally delivered in four 2-part sessions:

Health Care Decisions
First responders and hospice and palliative care providers will discuss important decisions and documents necessary to ensure the medical care you want, or don't want, at the end of life.

Funeral and Memorial Decisions
Representatives from a local funeral home and cemetery will discuss the benefits of planning the details of your remembrance. and the multitude of options available.

Let’s Talk: Advance Care Planning 101
Start the vital conversations with your loved ones by first thinking about what matters most to you, especially when it comes to serious health issues. Then, learn about the importance of having a clear plan for your care if you are unable to speak for yourself. Representatives from the Fox Valley Advance Care Planning Partnership will be available.

Make It Legal!
Hear from a local elder law attorney about how to prepare legally to protect your legacy. Then, complete or update your Advance Directive and Power of Attorney for Healthcare with assistance from certified advance care planning facilitators.
